Below are forms needed for registration.   Annual Registration must be completed before starting at Da Vinci Charter Academy.

• Welcome Back Letter
• Review the Student Handbook
• DVCA Technology Contract Addendum (in addition to District agreement completed through Annual Registration) 
• University of California Eligibility in the Local Context form - Grade 11 only
• Comprehensive Sex Education Opt-out (optional) - Grade 11 only
• Da Vinci Booster Club Welcome Letter and Donation Form Spanish Da Vinci Booster Welcome Letter/Donation Form
• National School Lunch Program - forms available at www.djusd.net
• Purchase Da Vinci High yearbook or senior ad at yearbookforever.com.
• Senior Google Form
• Optional DJUSD ASB cards can be purchased at the Blue Devil online store.  All users must first create an account and connect the account to your student.
